Thank you for expressing your interest in becoming an approved carrier for Box Logistics LLC. 

To have an approved carrier status, you must:

1. Provide your MC or DOT#.

2. Companies must possess active Common or Contract Authority.

3. Safety Rating must be Satisfactory or None.

4. Demonstrated financial stability or proof of factoring (Notice of Assignment).

5. Submit an electronic W9.

6. Our Broker-to-Carrier Agreement must be signed by authorized representatives

7. Safety Rating must be Satisfactory or None.

8. Furnish a Certificate of Insurance meeting the minimum requirements outlined below. Certificates should be submitted by your insurance producer (agent):

   • $100,000 Cargo Coverage (per occurance)

   • $1,000,000 Auto Coverage (per occurance)
